How does orthodontic insurance work?
There are many different insurance provides and insurance plans differ quite a bit in how orthodontics services are covered. Generally, each individual has a lifetime maximum benefit dollar amount for orthodontic services. This benefit is paid as a percentage of the orthodontic fee (diagnostic records, initial fee, monthly payments, etc.) until the maximum benefit has been reached.
In most cases dental insurance does not cover the full cost of orthodontic care. It is designed to reduce your cost but does not eliminate it completely. Therefore, please understand that you are fully responsible for all fees from our office, regardless of your insurance coverage.
Your insurance is a contract between you, your employer, and the insurance company. You will be expected to make payments directly to our office. The insurance company will reimburse you directly according to the terms of your contract with them.
Filing Claims
For your convenience, we will submit your insurance claim forms pertaining to the charges for care rendered in our office and furnish all of the necessary information to your insurance company to help you obtain the full reimbursement that you are entitled to receive. However, please remember that our primary financial relationship is with our patients or their families and not with the respective insurance companies.
Please complete the insured or employee portion of your form and sign where indicated. We will then file your claim forms for you.
If your insurance company requires verification of continuing treatment, we will help you to provide them with proof of payment. You may send them a copy of your canceled checks or cash receipts. In addition, we can provide you with a copy of your account record, which will reflect all payments made to date.

How are my payments to Cordray Orthodontics made?
As a service to our patients we offer a number of affordable payment options. We request that your initial fee be paid at the start of treatment (the day that the braces or appliances are placed). The remainder can be paid in monthly installments over the course of treatment. The frequency of visits has no bearing on the monthly payments. The payment schedule is merely a convenient way to help you meet your total financial obligation. We do not charge interest on accounts that are kept current. (However, if a payment is not received within 30 days of the due date, a 1.5% per month or 18% per year delinquent service charge will be assessed on the payment that is late)delete?
